Myers-Stevens School Insurance Information & Forms

Every student-athlete is required to have insurance coverage to participate in any sport at Beaumont HS. For those who do not have coverage through an employer or other source, Myers-Stevens offers student accident insurance coverage. It is also recommended that these insurance options be evaluated as supplemental insurance in addition to employer-sponsored health insurance. The brochures are available below in both English and Spanish versions. The company offers a wide variety of plans and coverage. If your student-athlete is interested in playing football, you must select the "Interscholastic Tackle Football Accident Plan", and evaluate the other plan coverage that may meet your needs. If you need to purchase any coverage with Myers-Stevens, please read the letter and brochure attached below, and visit the Myers-Stevens website. Your student's coverage will begin once you have completed the online process, and you will be sent confirmation of your coverage, which must then be included on the emergency form in our athletic packet, and then turned in to the athletic department office.

The Riverside SPORT Clinic is the Inland Empire's premiere orthopedic clinic. The flyer below offers information for your student-athlete to be seen and have a diagnosis of their injury. This can be an important step to have in place when you begin seeking treatment and/or referral through your primary care physician. Please see the flyer for all days and times available in the event of an injury.
